Copeland Shoulder

La protesi Copeland di spalla è una protesi di superficie non cementata, sviluppata per imitare la naturale morfologia della testa omerale e ripristinare la normale anatomia nei pazienti artritici.

È la protesi originale di superficie per spalla, nata 20 anni fa¹, oltre che la soluzione di prima scelta nel mondo.

Design

Il design rivoluzionario della testa omerale di superficie Copeland™ presenta un esteso bordo profilato per press-fit circonferenziale e uno stelo rastremato a sezione cruciforme, per assicurare un saldo fissaggio primario mediante press-fit.

Alcune delle caratteristiche principali sono:

• Rivestimento poroso a pori chiusi e idrossiapatite per ridurre al minimo l'ingresso di frammenti dovuti all'usura e assicurare un fissaggio duraturo^2,3
• Rivestimento poroso in Titanio per un'eccellente biocompatibilità²
• Articolazione in Cromo-Cobalto e polietilene Arcom™ per contenere al massimo l'usura
• Sistema di caricamento dall'alto
• 8 misure di protesi disponibili per imitare il più possibile la normale anatomia della spalla
• Facile conversione in componente con stelo:
  • rimozione del solo componente di superficie e sostituzione con impianto a stelo; oppure
  • resezione della testa omerale e del componente e successivo impianto a stelo

Strumentazione e tecnica chirurgica

La tecnica chirurgica di Copeland per la spalla è mini invasiva ed è mirata ad una preservazione ossea.
Questa tecnica chirurgica si basa su un processo di preparazione dell'osso in 3 fasi:

1. Determinazione del centro della testa omerale
2. Alesatura per definire e ripristinare la morfologia della testa omerale
3. Creazione del foro per lo stelo centrale

La pratica strumentazione, identificabile mediante un semplice sistema di colori, agevola ulteriormente il lavoro dell'equipe chirurgica.

Tra il 1993 e il 2002 un campione di 209 spalle sono state sottoposte ad artroplastica, di cui:

• 180 interventi di emiartroplastica
• 29 interventi di sostituzione totale della spalla

Risultati

• Nessuna radiolucenza omerale dimostrata in questo campione
• Tasso di sopravvivenza del rivestimento in idrossiapatite del 98% a 10 anni
• L'analisi di Kaplan Mier suggerisce un tasso di sopravvivenza della protesi del 96,4% a 10 anni
• Il tasso di revisione per la protesi Mark IV con rivestimento in idrossiapatite, presentato nel 1993, è del 2,6%. Inoltre, le revisioni in OA sono dello 0,7%, il che corrisponde ad una sola protesi revisionata!
• L’emiartroplastica e l’artroplastica totale non presentavano significative differenze in termini di esito funzionale in questa serie

Conclusioni

• Finora non è stata riportata alcuna radiolucenza dall’introduzione del rivestimento in idrossiapatite
• I pazienti OA continuano ad ottenere i migliori punteggi funzionali
• Il 16% dei pazienti in questa serie è stato sottoposto ad artroplastica totale della spalla, provando che è possibile sostituire il glenoide con una spalla Copeland!
